Golf player Handicap Calculator in ASP VB

Hi I am looking for a code to calculate Handicap from a given GOLF player  in ASP (Visual Basic). I could start from scratch , but the thing is I am out of time to finish this project.

If someone has done something similar and has an existing code or can share a link to one please post it here.

 I am looking for something similar to

One more quesiotn. What do you mean by pass in?
I understand the formula but not the actual fields i need on database.
