Need photoshop document

I need to convert a ms-word .doc document (2pages) into a photoshop document.

It's a  comment card for a restaurant.

I also need the logo to be in the background on the 2nd page (just in the bottom portion where there is white space, I couldn't do this with Word)

If you have Adobe Acrobat you can print from Word to a PDF file and then open that file whit Photoshop. Once it is opened in photoshop you can save it as PSD file.
For the logo, you can duplicate the layer in photoshop and then clean up the rest, that will leave only the logo on a diferent layer and you can position it where you want,

Yes i have converted the word to pdf...I dont have photoshop ,Can u please help to get the other work done...

Do not use the printer icon, instead go to File\Print and when the Print windows open, in the printer dropdown menu select Adobe PDF. I am attaching a screenshot.
