Ad Impressions - How is an impression logged?

Hi everyone,

How is a single Ad impression being logged?

Does 1 impression means:
 
- The ad appears on a page one time. If user "A" refresh the page X times. The ad impression will be X times.

or

- The ad appears on the page for user "A". No matter how many times user "A" refresh the page, it is still consider as 1 impression

or

- The ad impression is logged by unique visitor?

Is there an industry standard for these calculations? I can't find any in the RFC specifications

1 impression is 1 load of the ad, refresh the page impression increments by 1

If you view the page 100 times thats 100 impressions.
