auditing sql database users roles permissions

I am looking for a method to create a report that will have the following

Login Name, Login Type, Authentication, Database(s) user had access to, Permissions/Roles to each databaes, Server Role

Example

SA   User   SQL   Sales_DB   dbowner   Sysadmin
