How does normalization work

I am working with an existing db called Workmens Comp which I will upload to you. I am currently running XP/Pro SP3 using Access 2003.  I have only one table and one form. The form consist of 5 different types of records which are Patient, Employer, Physician, Adjuster, Field Case Manager & Telephonic Case Manager.  I've also created some command on the right side of the form for quick reports.

Everything is working fine except when I try to retrieve a particular record for example Field Case Manager, I am not able to type in the Last Name and click a command button to retrieve this information.

My goal is to be able to retrieve all information for a particular record just by typing in the last name and click search.  I will greatly appreciate your assistance in this matter.  Thank you.
