Legacy ActiveX component radio buttons showing black background sometimes, any ideas?

an activeX component has some radio buttons and text boxes, occasionally it shows black background, see attached. works normally but some users see black. at the same time on two different machine's it can show one normal, one black, both have the same .cab versions.
any ideas? does this happened with any person?

Visual Basic Classic
