Yes he can save it to the desk top and open it from there just not in outlook
Please check the file association before attempting this.

I had a user who was unable to open attached images and below was the fix:
1. Start, Run, "REGEDIT"
2. Edit, Find, Find What:"OutlookSecureTempFolder" (without the quotes)
3. Copy the folder in the key's data (Example: C:\Documents and Settings\%USER_NAME%\Local Settings\Temporary Internet Files\OLK#\)
4. Start, Run, Paste the folder from step #3
5. Delete all the files in this folder
6. Restart Microsoft Outlook 2003
You should now be able to open the PDF
Just found this MS article: http://support.microsoft.com/kb/305982
It's a less destructive approach. (although you shouldn't do any harm deleting a temp folder)

Outlook 2003
If you are using Outlook 2003, follow these steps:
1. Click My Computer, and then double-click drive C.
2. On the File menu, click New, and then click Folder.
3. Name the folder temp0.
4. Click Start, click Run, type regedit, and then click OK.
5. Locate and then click the following registry key:
      HKEY_CURRENT_USER\Software\Microsoft\Office\11.0\Outlook\Security\
6. In the right pane, double-click OutlookSecureTempFolder.
7. In Value Data field, type C:\temp0\ and then click OK.
8. Exit Registry Editor.
9. Restart the computer.
Thank you.
