Users Receiving SPAM from themselves

Hello,
One of my clients is having a serious SPAM issue where SPAM is coming from a user to the same users, example Joe@domain.com received SPAM from Joe@domain.com... I have his company going through third party SPAM filtering which is catching a lot of SPAM. I also have Trend's Anti-Spam software running on the front-end and back-end servers, and I am using the IMF that comes with Exchange 2003.... A lot of users are getting these emails from themselves and I am stumped as to where they are coming from. I thought it might be coming from and internal or remote user with a virus so I scanned had every computer and server that is on that network or connects to that network... There are no open relays on the servers and at the router we only allow SMTP from the SPAM filtering company&  I took all the valid users from the domain and added them to the white list in Trend, I then blacklisted the domain& Im not sure what else to do& Another issue is if we tighten the policies to tight then one or two emails wont go through and the client freaks out& Is there a device that I can put on the network to filter not only all incoming and outgoing  SMTP but also internal? Oh and they are a medical company so we cant scan the body for bad words like breast and penis&.
