Why do operators run backbones using slower SDH speeds and GPON for their access networks?

Many operators use SDH technology for their backbone networks.  Even at STM-256 speed of 42,8Gb how can it provide enough bandwidth for thousands of subscribers who have a GPON access network connected to the backbone, where they receive 2.4Gb downstream?
