How to apply form entry data to a printable image...?

I have a page with a simple form (first and last name).  I want to open a page with a coupon (jpg image) where it shows their name on the coupon (from the form data) and let them print the coupon with the image and name.

How do I do that?

    Do it the same way you would create a captcha, only put the name on instead of a random word.
