I would still be pointing the finger at the mobile device. The items have probably been touched in a way that makes them look like a duplicate. <br />
You haven't said what the device is, but I would reset it and then remove the duplicates. <br />

Does anything else touch the mailbox? CRM for example? They can cause this kind of duplication when they touch the object. <br />

I have a client that has tens of thousands duplicated entries after a certain date in their outlook calendar. If i look past this date (Oct 2007) outlook runs very slow and it take about 5 - 10 mintues to respond.<br />
We have tried programs that removes duplicated events, it did remove them but all the events came back.<br />
Any suggestions about how these are coming back? He does have a mobile sync'ing with exchagne, but i have confirmed that the server wins, so it should be replicating what is on the server. The issue was there before we sync'ed the mobile.
