Unable to format ScanDisk 1GB micro SD

My problem started when I transferred pictures from my hard drive to the micro SD.  All of the pictures on the disc were listed with the same ddd.dddd filename.  I decided to format the SD and start over.  I went to "my computer" and tried to open "disc M" where the SD was located.  A message came up stating that "the disc is not formatted, do I want to format now"  If I chose "yes" and attempt to format, I get a message stating that "the format could not be completed".  If I chose "no" nothing happens and the message disappears.  I switched computers and the exact same thing happens. What is going on and what do I do?

    Try to format it in Disk management.  Right click My Computer  > Manage > Disk management on the left > Choose the disk to format on the right.  Right click on it to format.  follow the 4 steps, filling in the fields you wish for formatting options.  Let me know if that works,

    I get a message saying, "the format did not complete successfully"  however the computer management bar says the disc is healthy and that 100% of space is free.

    You get that error when you try to format from the management snapin?  Do you get the same issue on other SD readers?  This may be a reader issue. You may not have a second one to test.  Wal mart, best buy, or the local photo shop may have one.  

    Do you have a second SD card you could try to format?  do you get the same error?  Worst case, do you have a warranty on the card?
