Template / Set Margins for Microsoft Word - Specific Margins and a seperate text box

I'd like to have a 'template' (not sure if that is the right term).  For a standard American sheet of paper (8.5 * 11 inches).  I'd like a template (a thing to 'set margins') in Microsoft Word.

-The left side should have a  0.5" margin
-The bottom should have a 0.5" margin
-The top margin should be 1.0" margin
-The right side should have a 0.5" margin
-In the upper right corner there should be a 3" x 3" square margin that should be overlaid over the other margins listed.  

These areas would be unavailable for typing.

-Directly below the 3" x 3" square block there should be a separate block which can be typed in separate from the rest of the document, it should be 1.15" x 2.5"
-Directly below this "typeable" block should be another area unavailable for typing that is 0.33" by 2.5"  

I have included a diagram that will hopefully show what I am describing. It is not to scale.

    You can set up the margins with the Page Setup dialog and a text box (or frame) in a page header. The typeable box can be set in a number of ways depending on your requirements.

    If the box content is the same on each page (or on each page in a section), I'd do it as a header. If it changes on each page, I'd set it as a text box (or frame) and create it with a custom style. The attached sample file shows how to do the latter.
