AD &Exchange Migration

We are currently migrating users from one Windows 2003 & exchange environment to another using ADMT and exchange buitin migration wizard, we have migrated one site with success however when trying to migrate the users from in a different location we experience the following issues which I think are related

1.Outlook clients cannot connect to the exchange using a succsesfully migrated account/mailbox, with the error "Cannot locate user in address list".
2.When logging in with a user account in the new domain after migrating a workstation windows (XP) create a new profile user.newdomain which happened in very few workstations in the first site that we migrated
3. users migrated cannot access file servers resources. SID history???

Please assist
