SQL Statement on a .PHP page?

Hi,

I have a query within a .PHP page (attached). I'm trying to format the query as follows:

If WarningLetterToBeIssued = Yes then DON'T display the record

OR

If DocsReceived = Yes AND DocsConfirmed = Yes AND DocsScanned = Yes then DON'T display the record.

What is happening is that only if all = Yes will the record not be displayed.

Any ideas how the query should be formatted? The attached code looks like a right mess, not sure if it's easier to read as an attached file, let me know if you need it.
