Multiple login prompts in Outlook Web Access

We have an SBS2003 box running Exchange as normal, set up and working fine for Outlook over Internet clients and Direct Push mobile users.  Outlook Web Access was fine until recently - now you log on as normal and see the mailbox displayed, but when you open a message or start a new message, you're prompted to login again (i.e. you see the OWA login page).  Once you login again, you're fine until the next time you open a window, then you have to login again.

The really strange thing is, it only seems to happen every OTHER time the server is rebooted.  I'm not 100% sure on this but am about to reboot to confirm.

Any ideas?

did you do any updates on the server? exchange updates?
did you rerun the CEICW wizard?

All Microsoft Updates have been applied.  If you mean "did the problem occur immediately after a Windows Update?", then I'm afraid I can't be certain, but it's certainly possible (we didn't have many users on OWA until recently, mainly it's Outlook over Internet and Direct Push, so we didn't notice this for a while).

I think I ran the CEICW wizard when this first occurred, but I'll do it again now anyway... no, same problem.

My initial description is slightly inaccurate - in fact you only need to log on twice, once you've gone past the second one it seems to allow you to get into multiple e-mails, until you log off again.

Well, it'll take a while to go through yet, but I'll check it all.

In the meantime I rebooted the server, and the problem is gone again, presumably until the next reboot.
some related eventlogs could also be interesting.

For the integrated and digest authentication enabled settings, which website are you referring to?  Default Web Site, Exchange, Exchweb, etc?

There is an ISA server (a separate machine) but I don't think that's the problem as we reboot the SBS2003 box to fix it temporarily - the ISA server doesn't need rebooting.  Also, it's ISA2004 so the article you referenced doesn't apply in any case.
Everything's still fine at the moment as we haven't rebooted since the last time the problem occurred.  After the next reboot, we should see the problem manifest itself again, and then I'll go through the various suggestions we have above - although I'm not sure I'll find anything relevant, as the only thing that will have changed is that the server will have been rebooted.
Ok, rebooted last night after some more Windows Updates, and after removing and reinstalling WSUS.  Problem now still not evident.  Perhaps the problem was related to the WSUS installation (which I removed because the SBS Management Console integration was broken) which integrates with IIS, so it's possible...
