Hi,
I want to design a jsp as follows.

Field1   Field2    Field3   Field4   Field5
Field1   Field2    Field3   Field4   Field5
Field1   Field2    Field3   Field4   Field5
Field1   Field2    Field3   Field4   Field5
Field1   Field2    Field3   Field4   Field5

Can any one tell me how to design this JSP and also ways to insert and update the jsp.

Just use an html table and add every line to it in a cycle.

Are you using any frameworks like struts or any taglibs?
If not may be you think about using DisplayTag

http://displaytag.sourceforge.net/1.2/

There are many ways and means to come up with a insert/update UI.
