WSUS several sites

If I install multiple WSUS and make one of the primary and the others will be Replicas, will Replica server download all the updates that the primary have, or will they just download the updates that clients that are connected to the server needs?

WSUS Replica servers are quite literally that - Distributed replicas to ease traffic over your WAN. If you have your primary/administration WSUS server set to only download updates once they are approved, your replica servers will pull down those same updates from the primary server (or wherever you specified their update source to be), once it inherits the approval status of the update from the primary/administration server.
