I don't even know where to begin looking for this problem solution.

Only ONE client out of hundreds is having this problem. Whenever she uses a distribution list she's created it adds random recipients to her outgoing message.

I have completely removed her contacts lists, distro lists and even completly replaced her computer.

I'm running exchange 2007 on server 2008 and I don't know what other info to give you than that,

search for and delete her outlook client name cache,
http://support.microsoft.com/kb/292928
and you can also exit form outlook, find and delete outlook.nk2 file then restart outlook,
