aspnet_Users table always showing duplicate records with different applicationID

For some reason everytime I create a new user on my website my aspnet_Users table always ends up showing 2 records for that user with different applicationID value.

Any clues? web.config supplied

Yes... the problem lies in this part of your Web.Config:
 <authentication mode="Forms"> and the "name" and "path" values...

On your "Production Server" these values must both be "/"... (On your development server they must be "/AppName"
Here's an example of what mine looks like...
