How can i design a self generating form using php/mysql/javascript

I am just trying to get some ideas right now

my boss wants me to create a web app were the users can create their own application bassically, that ties to our database.  Our form/application is pretty straight forward initially, textboxes, radio buttons, dropdowns, etc requesting Name, Project Title, project type, details/description, etc.  It also has an upload section/function.  the user submits the request, data sent to db, then a confirmation screen appears and email is sent. The user can then go to other pages to view list of projects and corresponding detail and then approve/deny project requests.

Where do i start investiging how to code this, are there in free resources that can be easily manipulated.  as I said the initial part of the form is fairly basic, but its the other functionality that I have to be able to incorporate into any solution.

I have developed a custom solution in the past for processing forms, then storing in a DB and emailing. However, you can probably find what your looking for at the following website;
