I tested it locally. It works fine.
Now I want to upload this on Web server. As far as files concerned I upload all the files on server but which files of database I should upload.
I created a database using my site control panel. But it only allows me to create name of my database. What database files should I upload on my ftp?

if you have purchased SQL Server hosting along with your web hosting than you have to get ID/Pass and IP of SQL server, connect to that server via your enterprise manager, and export your data from your local database to your server's database.

here is a link which will help you to register your hosting sql server to your enterprise manager.

http://msdn.microsoft.com/en-us/library/aa198220(SQL.80).aspx

Note: you should first have server's IP or URL/Id/Pass
