Citrix Presentation Server 4.0 upgrade to Xen Apps

I'm in the process of consolidating our Citrix environment and I'm now looking at moving our farm to XenApps. Is there a "direct" upgrade path from PS4.0 to XenApps? I've found documentation on PS4.5 but nothing really based on 4.0

Are you going to move to XenApp 5 on Server 2003 (AKA Presentation Server 4.5 with Feature Release 1) or XenApp 5 on Server 2008?

Then it is a fairly simple migration.  

How many servers do you have?

What are you using for your data store?  Access, SQL, MSDE, SQL Express, Oracle, DB2

Is your cloud always on? With an Always On cloud you won't have to worry about downtime for maintenance or software application code updates, ensuring that your bottom line isn't affected.

Current position:
4 Citrix servers -
NTCitrix - Win 2000 - Master with SQL Database
NTCitrix1 - Win 2000
NTCitrix2 - Win 2003 + Uniprint
NTCitrix3 - Win 2003
 
We would like to decommission the Win 2000 servers and move the master database to one of the 2003 ones. Then to upgrade the other 2 servers to XenApps for 2003.

The licence server is on a separate server which does not run Citrix.

Citrix does not recommend that SQL Server be installed on a XenApp server.

BACKUP YOUR SERVERS AND DATA STORE FIRST!!!!!

You can safely uninstall all the Citrix components from the 2 Win2000 servers IF this is a FULL SQL Server install.  If it is MSDE or SQL Express you will need to move that to another server first.

Install XenApp 5 for Server 2003 on one of the 2003 servers, reboot and it will become the data collector.
Install XenApp 5 for Server 2003 on the other 2003 server and reboot.
On the Win 2000 server with SQL Server on it, I would recommend leaving it alone.
On the other Win 200 server, wipe it by loading W2K3 and doing a quick format of the drive and then install XA5.

Citrix does not recommend or support upgrading the OS of a server with XA installed.
I don't like to upgrade the OS of a server with installed software on it (i.e. SQL Server).

Oh yeah, the Citrix License Server will need to be upgraded to 11.6.1
