moving sms 2003 from SQL 2000 to sql 2005

I have a server with SQL2000 and SMS 2003 SP2.  (let's call it SERVER A)
it's the only SMS server.
I have another server with SQL 2005 (SERVER B).  
We are in the process of moving all the databases from SERVER A to SERVER B
I don't know the process to move the SMS database.
Anyone can help me and give the procesure to move SMS
The servers don't have the same name and I can't rename SERVER B
Let me know if you need more details

It would seem MS has outlined the process here.
