Well, after I posted the question, I got all the API code from Paypal support. Let me know if you want me to send you their sample code, which works perfect :)

It is a 43K ZIP file.

classic ASP
==========
Anybody know how to add a description to the DoDirectPayments for classic asp implementation? Their sample code that I use is below:

            .DoDirectPayment Request.Form("amount"), _
                  Request.Form("lastName"), Request.Form("firstName"), _
                  Request.Form("address1"), Request.Form("address2"), _
                  Request.Form("city"), Request.Form("state"), Request.Form("zip"), _
                  Request.Form("creditCardType"), Request.Form("creditCardNumber"), Request.Form("cvv2Number"), _
                  Request.Form("expDateMonth"), Request.Form("expDateYear"), Session("ActionCodeType")

PHP
=======
I've done it with PHP by adding it to the end of the string and it works great. See below:
$nvpstr="&PAYMENTACTION=$paymentType&AMT=$amount&CREDITCARDTYPE=$creditCardType&ACCT=$creditCardNumber&EXPDATE=".         $padDateMonth.$expDateYear."&CVV2=$cvv2Number&FIRSTNAME=$firstName&LASTNAME=$lastName&STREET=$address1&CITY=$city&STATE=$state".
"&ZIP=$zip&COUNTRYCODE=US&CURRENCYCODE=$currencyCode&CUSTOM=$custom&DESC=$desc";
