internal domain: internal.local
external domain: mydomain.com
app server with IIS: apps (having internet facing IP)

So, internally, my internet facing app server name is: app.internal.local
I want to have the external URL to this server shown up as app.mydomain.com

Is this something that need to be done at the web hosting side or something that I can do internally at my internal DNS server?  Thanks.

You need to register a domain first

now, either you can have your own dns server for the domain, or you can use domain registraiton company's dnsserver.

if its domain company's dns server, then you will have to request them to create a sub domain or may be from domain control panel you will have an options to create sub domain ...

or if you do your own dns server and point this dns server with this domain, then you would be able to create subdomain in your dns server

if you just  buy domain, then its unlikely domain registration company will give you chance to create subdomain, but you can always check with them that if you can create a sub domain
