OSD fail to run task sequence after password for the boot cd is entered

We hired a consultant to setup our SCCM server which includes the OSD process.  It looks like it is integrated with MDT.  Basically the imaging process that has been working well for us for the past 9 months goes like this: we use a PE boot image to boot, then we enter a password for the boot image cd, after this step, it hooks on to a web service where we enter the computer name and the collection. After this step, we're presented with a list of availabe TS to choose from and the imaging process is automated from there on.

This morning our imaging process fails and we notice that DP manager reports an error on one of the DPs that contain the package MS CCM framework.  We found out that the problematic DP was brought out of commission yesterday because we closed down one of the clinics.  So we deleted the package, re-created the package, copy the package to the working DPs, advertise it to the same collection but after that we still experience the same issue.  The issue we have is that we get a fail to run task sequence message right after we enter the password for the boot cd.  

It doesn't seem like the it can hook to the web service where we can enter the computer name to register it with CCM database.  I am not really clear on what role the CCM framework play in the imaging process, but basically that's what happened. Do you know what possible go wrong here?  I did restart the server but it doesn't help. In additional, I also exam the smstslog, but to my limited knowledge, I can't find anything useful.

Sounds like a timing issue...how much time are you allowing between when you add the PC and then look for the task sequence?
You can try adding a sleep statement to the media hook as given below (http://social.technet.microsoft.com/Forums/en-US/configmgrosd/thread/95841ef7-3128-4e80-a590-86f648daafcf):

Do you have some type of access to the admin console for SCCM?  If so, I would check the All Systems collection from the console to see if the computer record is getting created.
If the computer record is getting created and put into the right collection, look at the task sequence advertisements for the task sequences.  Make a note of what task sequence adverisement is advertising to what collection.  I would then delete the task sequence advertisement and re-create a new with advertising to the same collection.

I deleted the MDT and ZTI packages that the OSD reference and recreated them, then copy to the DP and everything works well now.  The root cause I think was because the system team remove on of the clinic distribution point and cause the DP manager to freak out.

No it's still not working... Basically I get an error message "Fail to run task sequence: there are no task sequence available for this computer" after I entered the boot cd password. It had been working fine up until the past few days meaning that I used to be able to enter the computer name and the name gets added to the specified collection in the MDT script; then I'll be able to select a task sequence to run. I review the smsts log attached in the first post but do not see anything unusual. Is this the problem with MDT or is it the boot cd?

Thanks I'll try it out.  Is it because the time set to too short or too long?  I exam the media hook file currently on our system and the time is 30000 or 30 seconds.  The script that goes to the bottom of the file are currently not there though.  Why do I need to created the boot disk file?  Is the media hook file copied to the boot disk or is it called from some process from the boot disk?

Time would be too short, but the one you posted is already 3 times the value I gave, so my fix isn't going to help you.
After entering in the computer name by the hook, fire up the admin console to see if the record is getting created.

The problem is it can't get to the part where I can enter the computer name.  It fails right after I enter the password for the boot CD.  I am trying to trace exactly where the problem occur, but I don't clearly understand the whole process.  The smsts log file I posted above does not tell me anything useful.

That's interesting, because the log above says:
Netbios name: TEST4 TSMBootstrap 4/30/2009 1:32:10 PM 1408 (0x0580)

Which seems to indicate that knows that this PC is TEST4...which would either have to be sent in via WinPE manually or come back from SCCM as an existing record.
I've done SCCM OSD without the MDT or the hook. All the ZTIMediaHook does is allow you to add a PC to a collection without accessing the admin console to add the record manually.
No assigned task sequences would indicate to me that either the task sequence isn't being advertised, either at all or to the right collection or the computer record is not getting created.
Check Software Distribution>Advertisements.  Is there an advertisement for the task sequence that you are trying to run?    Try deleting and recreating the advertisement for the task sequence that isn't running.  You can make a new task sequence advertisement by right-clicking on the task sequence, going to Advertise and walking throught the wizard.
