how do i make an invoice (pdf) via php from a excel invoice please

i have an invoice made with excel and id like a customer to be able to download(or print) this file via pdf. i would like the info to be populated via php/msql.

i know how to use mysql and php and have the site all set up for that, my problem is the displaying the ability to download the pdf file thats populated with the mysql info from a excel template.
if its a saved pdf file and php populates the info thats fine too as long as it LOOKS like the file i have

stuff like the date,name,addy etc and services need to be filled in.
how does one do this please thank you in advance for any code and help you may provide

for simplicity sake something like $name,$street etc can be used as i know how to pull the info from a mysql database to populate the info i just need the way to make a pdf file and populate the  pdf with the info thanks
