Cisco CTIOS Cannot log in - timeout error

We have about four phones that we cannot logon to through Cisco CTIOS. We are getting a timeout error. I am also seeing errors in the log that says "ERROR - No AgentInstrument provided in SYS_INSTRUMENT_OUT_OF/BACK_IN_SERVICE and a connection reset by peer error. Can anyone help me?

Are you using CUCM for the phones?  It sounds like you are trying to use a phone that is either not associated with your JTAPI user account or you have not created the device targets on the PG.  Another thing to try if you are already associated with the JTAPI account would be to disassociate the 4 phones and then reassociate them.  

What version's of software are you running?
