Means that you passed the wrong number of items.
I.e. it seems as though you have prepared a dynamic query, but when executing you provided one item less than required.
That is just one possible explanation.
Post the query, and the solution might be obvious.

i want to read file content to database
is that mean the column field name is not match with the name in database?

You have not posted the query so it is hard to say.
No, it means that you have may have more columns, than values or the other way around.
(column1,column2) values (value1)
or
(column1,column2) values (value1,value2,value3)

Post the query and it might be clearer.

Alternatively, you may have values (value1,value2,value3), but the table into which the data is being inserted only has two columns.
