gvamsimba,

Typically, my report header will include a meaningful name for the report, in this case something along the lines of Patient Demographic Summary, the company logo, if the report has any parameters I also include them  here. For example if you have a date range you are reporting on I would say  something like

Patient Demographic Summary (larger text, bold perhaps)
--------------------------------------
Between StartDate and EndDate

As for the footer, I usually put the Page number, the Execution time (Globals!ExecutionTime)

In my line of business, a lot of our reports are Company Confidential, or Department Confidential, so I usually note that on the report header or footer as well.

Yes I use textboxes.

The expression for the date parameters... i'm assuming you have parameters named @StartDate and @EndDate

="Between: " Parameters!StartDate.Value + " and " + Parameters!EndDate.Value
