Make my Flash upload script work

I would like to use this free PHP/Flash application to provide website visitors with a progress bar for uploading images:
         http://fds2432.googlepages.com/in-a-flash.zip
         http://www.inaflashuploader.com/

But I can't get it to work!  

Could you please provide me with the code for a working demonstration?

I'll assume you've posted the contents of the zip file to a folder named 'uploader' on your website. You also need a folder for the uploads, they use dropbox, this folder will need write access for the webuser (sometime apache)

Can you post your sample code?

Also what type and how large a file are you attempting to upload?

did you create those folders? or modify the sample code?

what type of file? the sample will ONLY accept .jpg files

I did not modify the code.   But the code does NOT contain an HTML form for testing purposes.  

They expect users to create their own HTML form to use with their class however I do not know how to do this.

you just need to post this code where you want the uploader bar.

post your HTML code for the form and the php script to process the data
