Quick Create is Gone

In Microsoft CRM 4.0 I have a custom entity form with a lookup to select an account.  The account lookup lets me pick an existing account or click New, which gives the "Quick Create" account window.  However, it started giving me the full contact form.  Any ideas how to set this back?

I think you'll find that the Quick Create was a feature of CRM 3.0 that has disappeared from CRM 4.0. I've just tried repeating your scenario and I get the full form as well in my CRM 4.0 system.

It's definitely a 4.0 feature.  I've seen this feature, but I've never worked on or seen a 3.0 system.

Thank you very much - the reason I've seen it is because Stunnware's Filtered Lookup gives a dialog that has it.  I temporarily removed Filtered Lookup from two lookups on the form and that's why the Quick Create reverted back.
