Mass Producing Software Application

Hi, I'm not sure if this is the correct section for my questions, but I'll ask it and we can go from there...

We would like to have a software application we're developing burned onto CD and packaged into a small box for sale. The service would have to mass produce the final product. My questions are as follows:

1). What would a company that provides this service be called?
2). How would I find such a company?
3). Is there a "do-it-yourself" type of kit that we can use to get started?
4). If we go with a service, what are the basic rates for a CD and a box?

Lets start with these questions and I think that will give us a lot of guidance for how to proceed.
