From addresses not showing up in outlook

Hello,
 I have a client running MS 2003 server with exchange and using outlook 2003. Every once in a while, they receive and email with nothing in the "from" box but it is a legitimate email.  If they hit reply the email is in there.  Occasionally they have sent each other emails and the "from" is missing as well.

Is there anything in common about the emails that have this issue?  I seem to remember seeing this once, some years back, and it was specifically with emails being sent from a web form, or dragging n dropping an attachment onto an icon, or something weird like that.  I can't remember.  But the first step I think is, can you MAKE this happen if you try? You will probably have to contact the sender and ask them to re-send to duplicate the issue.  Then you can start narrowing it down to the behaviors/clients that might cause it.
