Change control procedure document

I manage a number of servers on behalf of clients, and I would like to present my clients with a comprehensive, rigorous change management policy document which describes things like when the maintenance windows are, what procedures need to be followed when making a change to a Production environment, who gets notified and when, what happens in the event of a production down event, etc.

I could probably write one myself, but it would take a while and I hate writing documentation from scratch.  I kind of need to have at least a working draft by next week.  Also, I don't want it to be a 400 page tome - I would prefer a simple, readable, document of just a few pages.  After all, this isn't rocket surgery.

Can someone either share a document they currently use, or point me in the direction of a document which can serve as a starting point?  I will obviously edit it to suit my particular requirements.

I know I am being somewhat lazy here, but hey - why re-invent the wheel.
