Explorer.exe does not load upon startup/login

From time to time, and intermittently across 3 locations, some users when the start up their computers do not get the start menu and icons on the desktop. In order to resolve this, then we've been asking users to click - ctrl/alt/del - task manager - click file/new task (run), and typing explorer which then produces the start menu and desktop icons. I was wondering if group policy can cause this issue, or if you could also advise me on the cause on the intermittent issue? Thanks!
