Valid Email hits Outlook Junk Mail Folder

I have at least one user where an email made it through my Exchange server filters and ended up hitting the end users mailbox, only to land in their Junk Mail folder.  How does this happen and how can I easily and systematically prevent this from happening for valid emails?

If you are in a Windows 2003/2008 domain and are using Outlook 2007, then you can control outlook's junk mail filters through a GPO, i.e. you can add known safe senders to a global whitelist.
