Is there anyway to tell the status of an email sent threw sql server?  If I query sysmail_allitems it shows an email as being sent, but i have a supplier that is saying they are not getting it.  So I'm wondering is it actually sent (ie. did it leave our exchange server).  Does this sent status mean yes, or does it mean it left sql server only?

It means it has left your SQL Server.
If your exchange denies routing of the mail, then it would show an error in the error log of database mail.
You should check your Exchange setting Junk Mail Filter.
