Outlook express 6 sends two emails.

I have two different people asking me why outlook express is sending two emails when they only send a single email. One of these is getting read receipts (two of them from different people they sent different emails to) and the other person gets phone calls. I have unchecked the "send messages immediately" and "reply to messages using the format in which they were sent" also switched the mail sending format from HTML to plain text. To my knowledge only emails with attachments, so I'm thinking it may have something to do with our server or the virus protection on the recipient end. Anyone have any ideas?

Check Sent Items folder. 2 copies there?
Send email without attachment to some external account of yours. 2 copies?
Send email with attachment to some external account of yours. 2 copies?

The sent folder shows one email being sent. But two emails (only when there is a attachment) are received.

I think I figured out the issue. The users were finding the file they want to send, right clicking and 'send to, mail recipient'  that pulls up a email and they type in who they want to send it to. When the same users send the same attachments the correct way (create a email and attach the file) they only send one email and recipient only gets one email.
