Why Drive Gen4 FC(IBM TS3100) - Offline many times on Backup Exec 12.5 ?

Hi everybody experts.
I have to the problems about to Drive Gen4 FC of IBM TS3100 on the Symatec Exec 12.5 it Offline many times . So i tried to restart services of Backup Exec it already as though working for a moment .  But untill to Schudule Backup it not working. So i has upgrade Backup Exec 12.5 Service Pack 3 the problem same it occur. Please tell me for solutions.

Check device manager on server THBACKUP01. Is the tape drive still listed under the Tape Drives section? Make sure there is no yellow exclamation mark on it.
In the Windows services, please make sure the removable storage service is stopped and disabled.

Check your system event log on server THBACKUP01 - check for events 7, 9, 11, 15 from the tape drive driver or the SCSI card or Fiber HBA driver. Event 7 would indicate a possible tape problem. Events 9 and 11 would generally indicate a hardware problem on the SCSI chain.
Make sure to update your drivers with the latest device driver pack you can download from the Symantec site. Also update drivers for SCSI or Fiber HBA card.

You also have a server NSTHM001 listed. Is that server supposed to see the same tape library and tape drive? If it is supposed to see it, is it going offline there as well? If it goes offline there as well, it could be a problem with the tape library or a tape drive. If it stays online there, it could be the SCSI path to the drive.

Because the answer it very well but haven't reason the information of driver the last support Service Pack.
