Passring values in jsp

Am I limited to passing one value from one form to another and if not how do I do that? this is what I have right now:

onclick="document.location.href='WASIPMForm.jsp?name=<%=applicationName%>'; "

I tried this and it seems it just passes the whole thing as the value of name:
onclick="document.location.href='WASIPMForm.jsp?name=<%=applicationName%>date=<%=createdDate%>'; "
