How do I make a online form?

I want to create a two page form. On the first page the user enters their name, number, and company name. After they enter that information they will click continue and it will go to page two. On that page they will enter dollar amount into a field. That number will be multiplied by a number that will not be seen. Then the user will click submit and it will email the info to me. How do I do this or can someone point me in the right direction.

    To do this you will need a server side language (like PHP, ASP, etc) to read the form input when submitted. It is not very feasible to do it in just HTML and javascript. Do you have a server side language on your website?

    If not, you could do it with javascript Querystring, but again, not recommended.

    Let me know what server side languages your website has if any at all.

    Great. In the mean time if you need any help learning about HTML forms, W3C Schools is a great website:

    They have a bunch of information on all things HTML and have try-it-yourself examples which make trying out the forms easy.

    Then depending on what server side language you have, they have examples of those as well.
