Document Toolbar Missing

In my Dreamweaver CS4 the Document Toolbar, which shows at the top with
Design  Code Split  Title [                  ]
is missing.

I cannot seem to find how to turn it back on.

Right click on any part of the tool bar underneath your open file tab.  Hit "Document"

The only view I have is:

File Edit View Etc.
Common Layout Forms Data Etc
Icons for Common
Filename.html

I cannot see an editing icon set and my html file is the last item before design or code input.

Okay, I found the icon, on the top line for Design, Code, Split, but I cannot drag it anywhere.
