    If you're using Visual Web Developer 2008 then it already houses the functionality to present a WYSIWYG (What you see is what you get) visual interface...
    If I'm understanding the issue Simply Go to File-> Create ->  New Website...
    Go to Design View and Use to Toolbar to Drag Labels and Images onto the Page...
    The Development Environment supports Visual Basic and C# for the code behind languages for server based web applications or you can simply create a Static HTML file...

    On a webpage if you manually want to insert graphics...
    for example use...
    <img src="" alt="myFavoriteLogo" />

    for Checkboxes use:
    <input type="checkbox" name="question1" value="Yes I like Green Eggs and Ham" />

    It does not seem a trivial process to do the checkbook.
    I am thinking, I do a table with the background image of a check.  Then drop few input fields on some absolute positions.  Vow, but sounds cumbersome.
    How to I drop a perforated line from top to bottom to show seperation of stub and check. sounds complicated.
    Is there any easier way? I wonder?
