SAS controller : DELL SAS 5/e HBA
Backup Software - BackupExec 12.5
Backup library : Dell PowerVault TL4000 Tape Library

The issue :

Every job fails with the following error message :

Storage device "IBM 1" reported an error on a request to write data to media.
Error reported:
The request could not be performed because of an I/O device error.

Have you ran a diagnostic on the drive and tape library? Even though it is a Dell, the tape drive itself may be an IBM. There is usually a problem with the drive when you get this error.

The solution mentions replacing the tape. If it is the tape DRIVE, then I object because in my first post, I mentioned that the error usually meant the drive itself was defective. However, the solution mentions the tech changeing the tape, but I cannot image a tech being sent onsite for just a tape and not the drive.

Sorry, my bad

You are absolutely right - it was the tape drive
