what could cause an email to be sent to some of the recipients more than 20 times

My wife forwarded an email to about 18 people, and a few of them reported getting the same email several times, as many as 20 times or more.  Yet some reported no problem.

What could possibly cause this?

We use Outlook as a mail client.  We have up to date Zone Alarm Virus scans and Spy Bot.

There was a 6M  .wmv attachment.  But it all passed our virus scans.

The mail goes through our own domain which is hosted by a fairly large and professional hosting company.

I've asked their tech support to look into it, but I'd like to become better informed on this.
