Idea needed: building a job application form

I have to work on an aspx page and have a job application form. Name, address, up to 3 previous employers/workexpereince, paste OR upload resume, salary question, etc.

What's the best way of doing this? i dont think having individual textboxes is good. Should I have like a grid (thinking about using Telerik controls) and have all this in a grid?

** for example, i have a block to display one previous employer information. I have a link next to it to add 3 more but how can I do this..what control do i need to use?

any ideas?
