Hi,
Customer feedback with Outlook 2007.
is there any way we can have some radio buttons and checkboxes in the body on an email sent to all Local domain users and after they do the selection and send back to me collect the reports from the mails to excel.?

You can create a survey based on a custom Outlook form (avishar's first link), but you will be better served by creating a simple web based survey and including a link to it in help desk emails.

What does an oft file have to do with a web based survey?

Sorry, I misunderstood. I suggested using a web based template. You are going with avishar's recommendation. I will leave it to avishar then.

1.Enter the recipients in the To box and subject in the Title box.
2.Pick a question type from the Type box.
3.Type the text of the question in the Question box.
4.Click Add.
5.Repeat steps 2-4 for the remaining questions.
When i clcik add there is no change at all.
Is there any permission setting before this.?
You need to first publish the form in the "Organizational Forms Library", The how-to for this is given in the link,

Web based template you mean a Html page and a link to the web page in the email.?
Yes, that's what I mean.
