Hi,

Customer feedback with Outlook 2007.

is there any way we can have some radio buttons and checkboxes in the body on an email sent to all Local domain users and after they do the selection and send back to me collect the reports from the mails to excel.?

Would be much better (easier and cleaner) if you just post the feedback form on a Web server and send everyone the link.  Having data collected in emails and trying to report on them would be very cumbersome.

You could also embed the form in email, but the target submit URL is on a Web server, so if users submit the form from the email, the answers go to your collecting server.  That way they don't even have to open a browser.  However there may be security complications with the approach.  Not sure on that since I never tried.

Outlook 2007 does not support the HTML form tag.  That prevents the use of an actual web form for this.  You could include the checkboxes/radio buttons in the body of the message, but retrieving the values when you received the responses would be difficult at best.  I agree with techhealth that the best solution is to implement a feedback form on a web server and include a link to it in the messages.  Another approach is to include links to a simple web service like page.  Each link would pass a specific value.  That would allow you to use custom graphics as buttons.  When a recipient clicked a button they'd be taken to a page that recorded their answer and thanked them for taking the time to provide feedback.  Can you give us an idea of the type of feedback you're looking to collect?

I will need a form that has 5 questions and asking employees to selectect 1 to 5 and send them back

You can create a survey based on a custom Outlook form (avishar's first link), but you will be better served by creating a simple web based survey and including a link to it in help desk emails.

Sorry, I misunderstood.  I suggested using a web based template.  You are going with avishar's recommendation.  I will leave it to avishar then.

Follow the instructions as given in that link, I have pasted the same below, does it work for you ?

1.Enter the recipients in the To box and subject in the Title box.
2.Pick a question type from the Type box.
3.Type the text of the question in the Question box.  
4.Click Add.
5.Repeat steps 2-4 for the remaining questions.
6.Adjust the question order with the Move Up and Move Down buttons.  
7.If you do not want the results to include a comma-delimited list of the answers, clear the Format Response in CSV format box.
8.Click Send to transmit the survey message.
The user receiving the survey must open it to answer it and will see the first question with a place to provide the answer (the exact control depends on the type of question), plus Next, Back and Finish buttons, as in a wizard. When the user clicks Finish, the form creates a reply to the survey sender and compiles the responses into the body of the reply message, in Q&A format. If the Format Response in CSV format box was checked by the survey sender, the responses will also be sent in comma-delimited format.

When i clcik add there is no change at all.

Is there any permission setting before this.?

You need to first publish the form in the "Organizational Forms Library", The how-to for this is given in the link,

"Prerequisite
To use this form, you must be able to publish it to the Organizational Forms library on the in-house Exchange Server. It cannot be used to collect data from recipients outside your organization"
