c# word add-in

I'm trying to build a word add-in in visual studio, and although I can happily create a new document and do all kinds of clever things with it using C# I want to be able to edit the current document programatically using my add in.

as far as i can tell, as I start up the add in in

private void ThisAddIn_Startup(object sender, System.EventArgs e)
        {

        }

i can access everything I want with the document.  I've got a ribbon button set that I'm going to be calling back from, and when these call backs happen, I can't find a way to reference back to the current application, only create a brand new app and document, which is not what I want to do.

any help, pointers or abuse would be welcome.
